Chronic Disease Self-Management Program
The Chronic Disease Self-Management Program (CDSMP) is designed to improve the self-management skills of people living with a chronic disease. The program was developed and tested by the Division of Family and Community Medicine in the Department of Medicine at Stanford University. The Washington Health Foundation assists rural communities and providers to establish CDSMP for their residents.

The program consists of a series of group sessions, including medium-sized classes and use of lay leaders. Trainers teach coordination skills for health management. Each class is highly participatory: mutual support and successes build the confidence in participants' ability to manage health while maintaining active and fulfilling lives.

Participants typically have a range of disease(s) and risk factors—types and severity of illness vary.
